Erasmus+ part-time studies: required documents

All candidates are required to submit documents and register in the USOSweb system.

Documents should be submitted in room 114 (to Ms Agnieszka Tokarzewska) or in the ERASMUS folder (in the cloakroom). We would like to remind you that only COPIES of documents are accepted.

List of required documents:

• application to the Dean of Student Affairs [DOCX];
• certificate of average for the previous academic year (issued by the Dean’s Office);
• letter of motivation (printed from USOSweb; Polish or English version);
• documents confirming the level of knowledge of the foreign language (confirmation of passing a university exam, language certificate, certificate of language proficiency, or, s a last resort, a statement of language proficiency with information on the level);
• List of subjects you wish to take at the host institution – proposed Table of Recognition [DOCX].
